The Care Management Registered Dietitian (RD) is part of the interdisciplinary team across the continuum of care providing primarily telehealth/telephonic education to our pregnancy members but may at times perform face to face education in the community.  The RD will provide nutrition and dietary guidelines based on National Standards of Care.  Supports/coaches members with goal setting and implementing self-care behavior.  Engages with the pregnancy members of the health plan with the goal of improving health care outcomes and appropriate and timely utilization of services across the continuum of care.  Supports breastfeeding education and navigation to resources.  Address nutrition related social determinants of health to support members in achieving optimal health and positive pregnancy outcomes.

Responsibilities:
  • Assess and evaluate our pregnancy members, coordinate care utilizing the most appropriate resources to support member’s needs.
  • Include member and family as appropriate.
  • Engage actively with the member PCP/OBGYN.
  • Assess nutritional needs and provides education based on National Standards of Care.
  • Assess and identify needed gaps in care metrics and navigate them to appropriate resources to close gaps in care.
  • Engage with the member in support of their treatment team to identify and establish attainable goals that positively impact clinical, financial, and quality of life outcomes for member.
  • Support and educate members regarding breastfeeding and lactation.
  • Assess Social Determinants of health and home safety and provide resources needed to support any identified gaps.
  • Assess the needs of members in the geo panel and align them with the appropriate member of the care team (behavioral healthcare manger, social worker).
  • Provide ongoing monitoring, evaluation, support and guidance to the coordination of the member's health care from preconception to post-partum.
Qualifications:
  • 3 - 5 years of experience required
  • 3 - 5 years minimum of clinical pregnancy education experience preferred
  • Bachelor’s degree required
  • Master’s Degree in Nutrition Science or equivalent preferred
  • NYS licensed Registered Dietitian (R.D.) required
  • Certified Diabetes Education certification or Diabetes education experience preferred
  • Prior health plan experience highly desirable preferred
  • Excellent verbal and written communication skills required required
  • Computer proficiency required required
  • Supports an integrated care model tapping into appropriate resources both internally and external to the organization required
