Outreach members both in person and telephonically for program engagement.  May perform in-person member visits in community settings, such as their home, hospitals and physician offices to introduce care management programs and validate member contact information.  Support members in collaboration with the interdisciplinary care management team to achieve healthcare goals and keep lines of communication open.  Provide access to community resources and assistance in removing barriers to care.

Responsibilities:
  • Perform outreach calls and visits to members to convey the programs offered.
  • Help members develop health management plans and goals and follows up on progress.
  • Coach members in effective management of their chronic health conditions and self-care. 
  • Help members in understanding their care plans and instructions.
  • Support non-clinical barriers to care interventions including making doctors’ appointments, referrals to internal and external resources, assist with transportation issues. 
  • Actively collaborate with care managers and support care management interventions set forth by licensed care team including discharge plan review and plan of care.
  • Identify all HEDIS measures for which the member is eligible, determine if member is already compliant and then work with the member to achieve HEDIS compliance for all eligible measures.
Qualifications:
  • Bachelor’s Degree
  • Community Health Worker Certification
  • 2 -3 years’ relevant professional work experience required
  • 2 years’ experience in health care industry, or other related activities preferred
  • Additional years of experience, and/or specialized training may be considered in lieu of certification requirements required
  • Proficient in medical terminology required
  • Experience working in a multicultural setting required
  • Fluency (verbal) in Spanish preferred
  • Excellent verbal and written communication skills required
  • Excellent customer service skills required
  • Verifiable good driving record or a reliable source of transportation required
  • Proficiency in MS Office (word processing, database/spreadsheet) required
  • Detailed oriented; strong organizational and prioritization skills required
