Qualifications:

• Bachelor’s degree  (Required)
• Master’s degree in social work (LMSW)  (Required)
• Certification in clinical social work (LCSW)  (Required)
• Active, unrestricted New York State License to practice Social Work  (Required)
• 4 – 6+ years’ relevant, professional work experience (preferably in occupational health or related field)  (Required)
• Case management and/or managed care experience  (Required)
• Health plan experience  (Preferred)
• Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs  (Required)
• Knowledge of NYS Workers’ Compensation System regulations  (Required)
• Knowledge of community resources access and ability to make appropriate referrals to internal/external programs that meet the patient’s needs  (Required)
• Ability to supervise social work interns  (Required)
• Strong organizing and prioritizing skills; ability to successfully manage multiple tasks/projects simultaneously  (Required)
• Must be detail oriented; ability to identify, analyze, and solve problems  (Required)
• Excellent communication skills (verbal, written, presentation, interpersonal) with all types/levels of audiences  (Required)
• Proficient with MS Office (Word, Excel, PowerPoint, Teams, Outlook, etc.)  (Required)
• Ability to screen and stratify patients who are appropriate for services; manage a caseload of patients who need case management; create and execute case management care plans and document per EH policies and procedures  (Required)
• Ability to engage and diffuse crisis situations; and to work effectively with patients, their families, professionals, and other
personnel from a wide range of cultural, social and economic backgrounds  (Required)
